
Medicare Part D is the prescription drug part of Medicare, sold as standalone plans by private carriers. Every plan has its own formulary, which is the list of drugs it covers and what tier each drug sits on, and its own pharmacy network with preferred pharmacies that charge less. Two plans with similar premiums can differ by hundreds of dollars a year once your actual prescriptions are run through them. Since 2025 there's a federal cap on what you pay out of pocket for covered Part D drugs each year, which makes the total-cost comparison more important than the premium. Going 63 or more days without it adds a permanent late enrollment penalty to your premium for as long as you have Part D. It's also worth rechecking every fall: formularies change on January 1, a drug you take can move tiers or drop off the list, and a preferred pharmacy can change. Clients who pair Part D with a Medigap plan need a standalone plan; clients on a Medicare Advantage plan usually have Part D built in and should compare the whole package instead.
To compare plans you need your Medicare number, a list of your prescriptions with dosage and how often you fill them, and the pharmacy you prefer. That's it.
